The My-Faults Book is a hardcover book of blank pages. It lets you write down all your faults, failings, problems, inadequacies, bad middos, bad thoughts, bad desires and bad actions so that you can look at your list and stay humble.
If you happen to be one of the many people who will be afraid to actually put many entries into the book due to fear that other people will read what you wrote and condemn you for what they find inside, nevertheless, just owning The My-Faults Book, even if you have to leave it completely blank, will help you avoid the very dangerous middah of gaavah (conceit), by helping you to remember that although in some ways you might be quite great, in a lot of ways you are also not so great.
But even though you might have to try to hide your entries in this book from people, you don’t have to worry about having to hide them from Hashem. Hashem already knows all your faults, since it was He who gave them to you in the first place.
Once you have recorded your faults and failings, you then need to work on yourself so you can get to the point where you can honestly and sincerely thank Hashem for each one of your faults and failings! That’s right, thank Him for them. And even though it might seem impossible to be able to honestly thank Hashem for your faults, it’s really not too difficult to do. You see, they are the things that keep you humble. And Hashem likes humble people the best.

ENGLISH TRANSLATION OF THE ABOVE QUOTATION
· (Rabbeinu Bechaya, Vayikra 1:1, s.v. shlomo) The middah (characteristic) of anavah (modesty, humility) is the middle point between the one extreme which is conceit and the other extreme which is lowliness. And behold, the midpoint in all the [other] middos is the middle way and the good way that a person should choose for himself and conduct himself in, and he should not deviate to one of the extremes, for that is a bad and bitter thing…. However, with the middah of anavah, we are warned with it that a person should deviate himself to the lowest end which is the middah of lowliness and not choose to remain in the middle point [of anavah], lest he comes close to the middah of conceit. For it [conceit] is a disgraceful middah which damages a person and disturbs a person in his body and soul from the temporary life and from the eternal life. And on this Chazal said in their mussar teachings, Be very, very lowly of spirit. And they doubled the word when they said “very, very” to teach that a person should turn himself to the lowest end, which is lowliness, because the middah of anavah is magnificent and excellent, and its advantages are enormous, and it is well known.
